Contractors

General Contractor:

 

The following must be submitted with your application:

 

  • A signed and notarized mercantile application. Forms are available to download at the bottom of this page.
  • Copy of Certificate of General Liability Insurance in the amount of $1,000,000.  The City of Long Beach is to be listed as the “Certificate Holder”.
  • Copy of Certificate of Workers’ Compensation  Insurance (C-105.2 or U-26.3) or a Certificate of Attestation of Exemption (Form CE-200) and Compliance with the Disability Benefits Law.  ACORD forms are not acceptable.
  • Copy of current Nassau County License issued by the Department of Consumer Affairs.  Not required for commercial-only contractors.
  • A list of five recent jobs which should include Name, Address, Type of Work Performed and Approximate Cost.
  • A Corporation or Partnership must include the names and address of all officers or partners.
  • A check or money order for $530.00 made payable to City of Long Beach.

 

Please Note:

  • All licenses are issued for a period of two (2) years or part thereof.  The license will expire on April 30thof the second year of the license.
  • Failure to include all necessary paperwork will result in rejection of your application.

 

 

Home Improvement & Misc. Contractors:

 

The following must be submitted with your application:

 

  • A signed and notarized mercantile application.  Forms are available to download at the bottom of this page.
  • Copy of Certificate of General Liability Insurance in the amount of $500,000.  The City of Long Beach is to be listed as the “Certificate Holder”.
  • Copy of Certificate of Workers’ Compensation  Insurance (C-105.2 or U-26.3) or a Certificate of Attestation of Exemption (Form CE-200) and Compliance with the Disability Benefits Law.  ACORD forms are not acceptable.
  • Copy of current Nassau County License issued by the Department of Consumer Affairs.
  • A list of five recent jobs which should include Name, Address, Type of Work Performed and Approximate Cost.
  • A Corporation or Partnership must include the names and address of all officers or partners.
  • A check or money order for $400.00 for Home Improvement contractors (Misc contractors see form for fees) made payable to City of Long Beach.

 

Please Note:

  • All licenses are issued for a period of two (2) years or part thereof.  Home Improvement Contractor license will expire on March 30th of the second year of license.  Misc. Contractors will expire on September 30th of the second year of the license.
  • Failure to include all necessary paperwork will result in rejection of your application.

 

 

Demolition & Excavation Contractors:

 

The following must be submitted with your application:

 

  • A signed and notarized mercantile application.  Forms are available to download at the bottom of this page.
  • Copy of Certificate of General Liability Insurance in the amount of $1,000,000.  The City of Long Beach is to be listed as the “Certificate Holder”.
  • Copy of Certificate of Workers’ Compensation  Insurance or a Certificate of Attestation of Exemption (Form CE-200) and Compliance with the Disability Benefits Law.  The WCDB-100 stamped waiver form is no longer valid.
  • Copy of current Nassau County License issued by the Department of Consumer Affairs.  Not required for commercial-only contractors
  • A list of five recent jobs which should include Name, Address, Type of Work Performed and Approximate Cost.
  • A Corporation or Partnership must include the names and address of all officers or partners.
  • A check or money order for $530.00 made payable to City of Long Beach.

Vehicle Decals:

 

Vehicle decals must be displayed on ALL vehicles used in conducting you business in the City of Long Beach.  The first two stickers are included in your renewal fee; each additional sticker is $12.00.  Please attach a copy of a valid registration for each vehicle

 

House Lifters/Movers:

Any contractor lifting or moving homes must have a City of Long Beach General Contractor’s License AND a House Lifting License from Nassau County Dept of Consumer Affairs.  No other types/classifications of licenses will be accepted for house lifting permits.

Applications where license fees are paid by check will be held seven (7) business days.

 

Applications where license fees are paid by Cash, Money Order or Credit Card will be processed within one business day.
